Advice to artists: if you're using a form page for your commissions, put your own name or contact info on it somewhere. Otherwise, when the browser window opens, we might not know who we're commissioning any-more.

That I agree with you, some peopple love to prank form on others and it is a time waster. Asking a username of those who signed a form is efficient and easy for finding detail well from clients.
